Overview

We are accepting new applications for the 2011-2012 School Year. If you are interested in sending your child to Pan American Academy Charter School, complete an application form and deliver it to the Main Office.

You’ll need the following documents:

A completed application form (Downwload English Version) Or (Descarga en Español)
Re-enrollment Letter (English) Or (Spanish)

The state of Pennsylvania requires the following for all students:
Your child’s original birth certificate (or passport)
An original proof of residence (current utility bill, mortgage or lease papers, etc.)
Immunization records
4 DPT (one must be after 4th birthday)
3 Oral Polio
2 MMR (both must be after 1st birthday)
2 Varicella (both must be after 1st birthday)
Your child’s original Social Security card (if child has one)
